Employment from home?

Usually when a program is being developed, all people working on it (namely designers and programmers) typically are all in the same building right? Is it possible to be on a part like 3D modeling but being able to work from afar? Like in another state? Or is that just too inefficient?

I'm aware of at least one program that was developed by a team based in Allentown, PA, Portland OR and Singapore. :D
