Camera Following Character -

I need the camera to follow my character as he navigates the world. As the character moves I point the camera at the character and if the camera is at least 10 meters away from the character I move the camera towards the character forward at 1.5 meters per second.
This technically works but there are some problems:
1.) How can I make sure the camera is ALWAYS behind the character. For instance when the character turns I'd like the camera to turn with the character.
2.) I want the camera to remain in the air so the user has a good perspective of whats going on in the world. However, once the character moves around enough the camera eventually winds up on the ground so you can't view the world all that well.
